Objective: To know and interpret the meanings of Primary Care nurses experiences with regard to the advance care planning at the end of life. Methodology: Qualitative phenomenological study, AGS-Northeast Jaén.14 interviews were conducted. Stages of data analysis: initial (transcription and global sense search), structural thematic (fragmentation into units of meaning, synthesis and grouping into themes) and interpretative. Results: Conversations for decision planning are scarce. Difficulties in helping to plan: lack of knowledge, lack of communicative skills, little experience, emotions, institutional problems, social influence and patients and family’s reluctance. Nurses maintain evasive, passive and, to a lesser extent, proactive attitudes. They think they can play an important role in planning. Conclusions: It is necessary a social change, to train the professional, to train him/her in communicative and emotional skills and to integrate the planning into the health institution in order to incorporate this process into the care of the end of life.
